Getting Acquainted
On your first visit to our office we will ask you for personal data and health background information. This information is designed to help us complete insurance forms and other correspondence.

Please keep us informed of any change in name, address or phone number. The health information provides a basic history to aid us in providing better dental care for you.

A thorough oral examination including an oral cancer check and periodontal screening is completed. X-rays will supplement the examination if they are needed. Discussion of your dental health and recommended treatment and cost estimates will be presented unless consultation study is required. A subsequent consultation appointment will be arranged at no charge should the additional study be required.

Dental Insurance

It is necessary that we work by appointment. Unfortunately, emergencies do occur which occasionally cause delays in our schedule. We feel that our patients' time is valuable and except for emergency treatment of another patient, you can expect us to be prompt. We, of course, expect the same courtesy. It is customary to bill for missed appointments, however, no charge will be made for rescheduled appointments provided 48 hours notice is given.

In a comfortable environment and at a price you can afford, where a team of professionals who are constantly working to provide you with the highest quality of dental care. We want you to be able to have the best dentistry has to offer and so, to accomplish this, we have worked hard to develop a financial plan to fit everyone's budget. Our hope is that now, everyone can enjoy the full benefits that modern dentistry has to offer.

Payment is due in full at the time of service where the procedure requires a dental lab, one half of the fee is required at the impression appointment.

We also accept Visa, Mastercard and Discover.

Other arrangements can be discussed and coordinated on a personal basis that will comply with your current budget forecast.

We accept most dental insurance. As a convenience to our patients, we will fill out and mail the insurance forms. Because of the many types of dental insurances, we ask that each patient become somewhat familiar with their policy regarding deductible amounts, yearly limits and procedure covered. By doing this, any confusion on coverage will be eliminated.

Please remember that an insurance plan is between the patient and the insurance company and that any problems with insurance payments must be between the patient and the company. The co-payment is the patient's responsibility.
